Requesting Medical Records

We are unable to process requests for a patient's medical records that are submitted through our website. To obtain copies of your medical records from Frye Regional Medical Center or from Frye Regional Medical Center- Alexander Campus, please call us at (828) 315-3115 to request a Release of Information form, which will allow us to assist you in receiving your medical record. We apologize for this inconvenience.
